High THC Cannabis and Psychosis: What the Evidence Shows
High-THC cannabis raises psychosis risk, most for daily users, teens, and people with a family history. Here is what the research shows and how to cut the risk.
Cannabis use is linked to a higher risk of psychosis. The link is strongest for daily use, for high-THC products, and for people who start in their teens. Most people who use cannabis do not develop psychosis.
A 2007 meta-analysis in The Lancet pooled 35 studies. Any cannabis use was tied to an odds ratio of 1.41 for psychotic symptoms. Heavy use was tied to an odds ratio of 2.09.

A 2016 meta-analysis in Schizophrenia Bulletin put the odds ratio for heavy use at 3.90 compared with people who never used.
A 2019 study in The Lancet Psychiatry followed 11,905 people across Europe and Brazil. People who used high-potency cannabis (THC at 10% or more) every day had about 5 times the odds of a first episode of psychosis. The authors estimated that daily high-potency use accounted for about 12% of first-episode psychosis cases across the 11 sites, and about 30% in London.

THC is the compound that causes the high and can trigger short-term psychotic symptoms. A 2004 study gave healthy volunteers IV THC. It produced hallucinations, paranoia, and disorganized thinking that faded within hours. CBD does not act the same way. Some trials show CBD reduces THC-induced paranoia. Ratios matter. A product with 30% THC and no CBD is not the same as a product with equal parts THC and CBD.
The prefrontal cortex and the endocannabinoid system are still developing in the teen years. Starting before age 15 is tied to an earlier onset of psychosis in people who develop it.
Confounding is one gap. People with a family history of psychosis may be more likely to use cannabis. Reverse causation is another. A 2018 Mendelian randomization study in JAMA Psychiatry found that genetic liability to schizophrenia predicted cannabis use. That points to reverse causation for part of the link. Potency and product testing data are also uneven, so dose estimates vary.
Get medical help if these last more than a day or two after use stops.
For these groups, the safest dose is none.
No THC dose is risk-free. Lower the THC percentage. Add CBD. Do not use every day. Wait until the mid-20s if you can. Track how you feel in the hours after use and stop if paranoia or racing thoughts appear.
