What the Evidence Shows

Research is thin. A 2011 randomized trial tested nabilone, a synthetic THC analog, in 40 people with fibromyalgia. Participants reported less pain and less anxiety than the placebo group. The trial was small. A 2021 review of cannabinoids for chronic pain found modest benefit and low-quality evidence. CBD alone has no strong trial data for fibromyalgia pain.

  2. Wait 2 hours after an oral dose before adding more.
  3. Raise the dose by 1 mg to 2 mg every 2 to 3 days.
  4. Log dose, time, pain score 0 to 10, sleep hours, and side effects.
  5. Stop and call a clinician for a racing heart, severe dizziness, or vomiting.

Side Effects and Cautions

Common effects: drowsiness, dry mouth, dizziness, short-term memory problems. THC raises heart rate. Cannabis can interact with opioids, benzodiazepines, and warfarin. Do not drive after use. Avoid cannabis in pregnancy.

What Is Unknown

No long-term trial of cannabis in fibromyalgia exists. The best ratio, the dose ceiling, and the effect on flare frequency are unmeasured. State programs differ on qualifying conditions. Some states list chronic pain. Others list fibromyalgia by name.
