Why immunoassays cross-react

Immunoassays use antibodies that bind a class of molecules, not one molecule. The assay for cannabis targets THC-COOH, the main urinary metabolite of THC. Other compounds with a similar shape can bind the same antibody and push the signal past the cutoff.

CBD and hemp products

That result is a true positive for THC metabolite, even when the user never used cannabis flower. Labels are not a reliable guide. FDA testing of CBD products found THC in some samples sold as THC-free.

Secondhand smoke

Studies of people in closed rooms with heavy cannabis smoke found THC-COOH levels below the 50 ng/mL screen cutoff in most cases. Secondhand exposure is an unlikely cause of a confirmed positive.

What to do after a positive screen

  1. Ask for the lab report with the assay name and cutoff values.
  2. Request a confirmatory test by GC-MS or LC-MS/MS on the same sample.
  3. Submit a list of prescription drugs, over-the-counter drugs, and CBD products with doses.
  4. Ask about a split sample retest at an independent lab.
  5. Tell the medical review officer about any efavirenz or pantoprazole use.

Detection windows

THC-COOH detection times depend on use. Occasional use: up to 3 days. Chronic use: up to 30 days, and longer in some cases. Body fat, dose, and frequency drive the number. No product clears THC from urine in a day.
